Good Faith Estimate for Health Care Services

Under federal law, individuals who are uninsured or who choose not to use their insurance have the right to receive a Good Faith Estimate explaining how much their health care services are expected to cost.

When you schedule services, you may request a Good Faith Estimate of the expected charges for your services.

The Good Faith Estimate is based on information known at the time the estimate is prepared and is intended to provide an estimate of expected charges. Actual services or circumstances may differ from the estimate.

The Good Faith Estimate is not a contract and does not require you to receive services from Allison Counseling, PLLC.

If you are billed substantially more than your Good Faith Estimate, federal law may provide you with the right to dispute the bill.
